APPLICANTS: PLEASE BE ADVISED:
1.
Submittal of Confidential Information.
By submission of an application under the requirements of this program,
every applicant acknowledges that they are subject to the following conditions:
a. METRO
will not accept an application for processing that includes
any personal information
that could be considered confidential and/or private information.
The following are examples of information that may be considered personal
and therefore not to be disclosed for privacy and security reasons: user
names, any kind of personal information such as birth dates, home
addresses, medical conditions, doctor diagnosis, social security numbers,
drivers license or personal identification numbers, phone numbers or
any similar kinds of information.
b.
Metro reserves the right to return applications, or portions of
applications, that are found at any time during the review
process to contain unacceptable information that is personal in
nature. If METRO returns all or
a portion of an application, the METRO's action
may affect the applicant's ability to meet required
deadline dates. METRO is not
responsible for the failure of an applicant to follow program requirements
which may result in missing program deadlines..
2.
Inability to Electronically Submit Application
Applicants who experience difficulty in submitting their electronic
applications to Metro within the final
24
hours prior to the June 2, 2008 deadline, shall have the
option of submitting the document in hard copy in order to meet the deadline.
Failure to follow the
instructions may subject an application to disqualification or rejection.
The electronic form builder has been developed to assist agencies in preparing
the FTA Section 5310 application. The form builder will assist preparers in the
